The National Health Service has published a report, identifying a list of events which should never occur in NHS funded services. 14 such events are identified.

Examples include wrong site surgery, although curiously not when that occurs as a result of a mistake in a referral letter: page 3. That seems a strange exclusion when the surgery site could usually be confirmed with the patient.

It has been reported that about 300 such events occur each year.
