The UK charity Action against Medical Accidents (AvMA) has published a proposal designed to identify patient safety lessons arising from compensation claims against health care providers.

The proposal is said to be similar to that arising in coronial hearings, where hospitals often identify system changes implemented after a patient’s death.

The proposal also seeks to confirm that open disclosure processes were adequately complied with.
